Use this form to order prescription repeats online so we can have them ready when you call in to the pharmacy. 

Please give us at least 2 hours notice if ordering repeats via this form. You may list your required repeat items individually, or use descriptions such as "all repeats due" or "all except..." 

If your instructions are not clear we will contact you to clarify what you need.
